Please fill out all fields below. To add a qualified member to your account, please contact a Member Services Representative during Membership Hours.
Membership cancelations or freezes must be submitted by the 15th of the month to go into effect for the following month. Example: Submissions for September must be submitted by August 15th.
All other changes to your membership should be submitted by the 25th of the month.
Accepted processes are electronic forms, in person form signed with a Membership Representative or mail. Not accepted processes are by phone or verbalizing your intention to a Front Desk staff member.
