Please note, routing number must be the one listed on your checks. Routing numbers from deposit slips are not accepted. Terms and Conditions(*) If the requested change in your membership agreement cannot be processed in accordance with your membership, our Accounts Manager will contact you to follow up.I understand that by clicking submit, this is considered my electronic signature and that I am the primary account holder and am authorizing Mt. Hood Athletic Club to make the changes that I have requested to my Membership Agreement. I also understand that club policy for cancelling my membership is the 15th of the prior month and that changes to my account are not guaranteed for the following month unless submitted by the 25th. Members may freeze their account by the 15th of the prior month (maximum of three months per calendar year). Buy out fees for Annual Agreements are due and payable upon cancellation.
